Early-Warning Checklist for Educators

Noticed something about a student? Check the signs you’ve seen and get a clear, printable summary to bring to your counselor or student support team.

Research on early-warning indicators points to three areas that often show up long before a student leaves school: attendance, behavior and engagement, and course performance. This checklist helps you organize what you’ve noticed so the team can talk about it. Nothing you enter is saved or sent anywhere. Closing or refreshing the page erases it.

Step 1: The student

Use initials or a first name only. Don’t enter last names, student ID numbers, or other students’ names.

Step 2: Signs you’ve noticed

Attendance
Course performance
Behavior and engagement
Life outside school

Check only what you already know. Don’t question the student just to fill this in.

Step 3: Add context (optional)

This checklist organizes your observations for a team conversation. It isn’t a diagnosis, a risk score, or a prediction, and it doesn’t replace your school’s early-warning system or referral procedures. Follow your district’s student privacy policies when sharing the summary. If a student may be in danger, follow your school’s safety procedures and contact emergency services (911) or crisis support (call or text 988 in the U.S.).
